Here's how it works:
Using the order form (available on the lobby table at the shul, or one can be mailed to you), circle the names of those for whom you would like to order.
Make out a check to KI for $5 for each name circled, and mail the check and your completed form to:

A lovely basket filled with gourmet goodies will be delivered to each recipient on Purim, with a card attached identifying each of those giving to that recipient.
THE DEADLINE FOR ORDERS IS FEBRUARY 14, 2010 –we will not be able to accept orders after February 14.

If someone is sending a basket to you, your name can be automatically added to the basket going to that person. If you chose auto-reciprocate, you will be billed after Purim for any additional names which were added to your order.
